This constitutes an order for one Minister from ULC Ministries of NW OH to legally sanction a marriage for the above Bride and Groom, referred as Client below (in the State of Ohio) (on the above wedding date), and to be a part of
CEREMONY_____, CEREMONY AND REHEARSAL_____.
Costs are $150. for “ceremony” and $300 for “ceremony and rehearsal”.
Deposits of $50 per service are non refundable but may be applied to a different date/event if requested in writing a minimum of 14 days before the scheduled wedding date.
All final payments are due in full one week before the wedding date listed above. The Client agrees to a $20 per day late fee for each day after the wedding date that final payments are not made. Client furtheragrees that all uncollected payments may be presented by Mark E. Simon for legal collection within 24 hours after the wedding date.
In the unlikely event that the assigned Minister is unable to appear due to accident, act of nature or God, Mark E. Simon will make every effort to find a qualified substitute replacement. In the unlikely event that a replacement cannot be found, Mark E. Simon will make a full refund of any deposits paid by the Client. Client further agrees that the refund will be the full extent of deposits paid to date and no further damages may be sought against Mark E. Simon for any reason.
Mark E. Simon has the right to subcontract to another service provider. The Client will be
notified who the scheduled service provider is prior to the event.
Mark E. Simon bears no legal or financial liability to any individual attending this scheduled
wedding who should be harmed or cause harm to another person or property for whatever reason or cause.
The Client accepts all responsibility for all guests, invited and uninvited, all injuries that they may receive, and any and all property damage caused by any guest, invited or uninvited.
This Contract contains all of the terms and conditions agreed upon by the Client and Mark E. Simon for all services listed in #1, and no other agreements, oral or otherwise regarding the subject matter of this contract or performance for the event stated on this contract, shall be deemed to exist.
